Student Profile Sheet for Guidance Counselors
All counselors require students to complete the Student Profile sheet that they will need as they work with students on college applications. Please download the file, type the information neatly, and print the form. Give one copy of the form to your counselor. Print a second copy of the form and give it to me if you want me to help you with college applications. Finally, print another copy and give it to the teacher (or teachers) if you need a teacher recommendation.

Recommendation Form
If you want me to write a recommendation for you, please download this file. Complete the file and give it to me. You will also need to give me the following:
- Copy of the Profile Sheet (See file above.)
- Any form or directions you were given by the college or scholarship agency
- A stamped, self-addressed envelope.
Please allow a minimum of 2 weeks for me to write your recommendation.
